With your Club Membership, there are:

• No claim forms
• No deductibles
• No pre-authorization requirements
• No pre-existing condition limitations
• No waiting periods

Program Limitations

This program is a discount plan, not a dental insurance plan. It cannot be used:

• In conjunction with another dental insurance plan
• For services required due to a worker’s compensation injury
• For treatment which, in the sole opinion of the treating doctor, lies outside the realm of her capability
• For referrals to specialists
• For hospitalization or hospital charges of any kind
• For costs of dental care which is covered under automobile medical

Program Guidelines

• Patient’s portion of bill is due day of service.
• If your plan lapses, you will be required to pay the one-time membership fee to rejoin.
• No refunds will be issued at any time if the participant decides not to utilize our dental plan.
